Community Care Plan (CCP), formerly South Florida Community Care Network (SFCCN), was introduced in 2000 as the first safety-net hospital-owned Provider Service Network. Community Care Plan, the health plan with a heart, serves members enrolled in Medicaid, Children's Medical Services Network and self-insured employee health plans.

Improving the quality of care to Medicaid recipients within a manged care system of delivery, to provide a high standard of health care and education to improve the health status of the community, to earn patient and customer satisfaction, enhance professional fulfillment, and provide public service.

We are a partnership owned by Memorial Healthcare System and Broward Health. CCP is fully accredited by the Accreditation Association for Ambulatory Health Care (AAAHC) as a Health Plan and is currently the only remaining Provider Service Network in the State of Florida.
