לאומית נוסדה בשנת 1933 במטבחו של ד"ר יעקב וינשל בתל-אביב, על ידי רופאים אשר פוטרו מעבודתם בקופ"ח כללית בזמן המאורעות האלימים שבאו בעקבות רצח ארלוזורוב.
מרפאתו של ד"ר וינשל הייתה המרפאה הראשונה של לאומית, שעברה מאז ועד היום דרך ארוכה.
בשנת 1940 קבע המשורר אורי צבי גרינברג כי תפקידה של הקופה הוא "לחנך ולרפא את הגופים" ואבא אחימאיר הוסיף כי "יש ללמד את היהודים להיות בריאים גם כשהם חולים".
מאז ועד היום עוסקת לאומית בקידום שירותי הבריאות, חינוך לבריאות ומתן שירותי רפואה מתקדמים לחבריה. עוד בשנות השלושים קבעו מייסדיה, כי ייחודה של לאומית יהיה במתן זכות הבחירה:
הבחירה בקופה, הבחירה ברופא והבחירה בשירותים הרפואיים.
ייחודיות זו נשמרת עד היום.
מנהלה הראשון של הקופה מר מלמדוביץ קבע כי הקופה תהיה פתוחה בפני כל דיכפין ודרש מהעובדים והמנהלים להתייחס לעמית בקופה כחבר ולא כמספר.
אכן, בדרך זו ממשיכה לאומית לצעוד, קופה אשר דלתותיה פתוחות בפני כל תושבי ארץ ישראל, העיר הקיבוץ וההתיישבות, עולים חדשים, תושבי דתיים, חרדים, חילוניים ובני המיעוטים.
