Over the last 8 years, SNC and its partners have provided a multitude of successful and nationally recognized web based healthcare solutions that serve the underserved and Safety Net populations. Our solutions encompass millions of patients from organizations including LA Care, LADHS, Orange County MSI, Orange County CalOptima, San Diego CMS, and Chicago MHN (an Illinois Medicaid Project). Our established infrastructure and years of healthcare experience position us to successfully meet our clients’ goals of improving timely and appropriate access to care for the Safety Net population. With a focus on providing solutions that are based on four fundamental pillars to providing care: 1. facilitate access, 2. track activity, 3. share information, and 4. refer using evidence-based data in real-time. This model has allowed us to positively impact the total ecosystem of care for everyone from newborns and new mothers to the adult Safety Net population.
